Family Intervention Questionnaire

The State Child Development Centre, Therapy Focus Inc and the Autism Association of Western Australia are working on a joint project which involves sending out questionnaires to the parents of children:

• who are school aged at the moment (ie: in years 1 to 12) and who are on a wait list for an autism spectrum disorder assessment;

OR

•  who were diagnosed with an autism spectrum disorder (Autism, Apserger Syndrome or PDD-NOS) any time since starting at school (ie: who were in years 1 to 12 at the time they were diagnosed) and who are now waiting for, or looking for additional, intervention services.

The information you provide will help to identify:

• what information and services families need,
• what resources are available but perhaps families do not always know about them or how to access them, and
• what areas need further development.

The project is keen to get feedback from families
 in both the metropolitan and country areas.
